Topical Therapies

- Topical Corticosteroids: These anti-inflammatory medications help reduce redness and itching. They are available in various strengths, and your dermatologist will prescribe the appropriate potency based on the severity and location of the eczema.
- Topical Calcineurin Inhibitors: Medications such as tacrolimus and pimecrolimus are used to suppress the immune response in the skin, reducing inflammation and flare-ups, especially in sensitive areas like the face and neck.
- Topical PDE4 Inhibitors: Crisaborole (Eucrisa) is a non-steroidal anti-inflammatory ointment that helps manage mild to moderate eczema by inhibiting phosphodiesterase 4 (PDE4) enzyme activity.
- Moisturizers and Emollients: Regular use of moisturizers helps to maintain the skin barrier, prevent dryness, and reduce flare-ups. Thick creams or ointments are typically more effective than lotions.
Oral Therapies
For more severe cases of eczema, oral medications may be necessary to control symptoms and reduce inflammation:
- Oral Antihistamines: These can help to reduce itching and improve sleep, though they are generally more effective for allergy-related symptoms than eczema.
- Oral Corticosteroids: Prednisone and other systemic corticosteroids can be prescribed for short-term use to quickly control severe flare-ups. Long-term use is generally avoided due to potential side effects.
- Immunosuppressants: Medications such as cyclosporine, methotrexate, and mycophenolate mofetil can be used to manage severe eczema by suppressing the immune system's overactivity. These are usually reserved for cases that do not respond to other treatments due to potential side effects.
Injectable Therapies
Injectable biologic therapies have emerged as a groundbreaking treatment option for moderate to severe eczema, particularly for patients who do not respond to conventional therapies:
- Dupilumab (Dupixent): This monoclonal antibody targets specific pathways in the immune system that contribute to eczema. It is administered as an injection every two weeks and has been shown to significantly reduce symptoms and improve the quality of life for patients with severe eczema.
Phototherapy
Phototherapy, or light therapy, is another effective treatment option for eczema. It involves exposing the skin to controlled amounts of ultraviolet (UV) light under medical supervision:
- Narrowband UVB Phototherapy: This is the most common form of phototherapy for eczema. It helps reduce inflammation, itching, and bacteria on the skin. Treatments are typically administered two to three times a week over several months.
- PUVA (Psoralen plus UVA) Therapy: This combines a drug called psoralen with UVA light to enhance the skin's response to light therapy. It is used for more severe cases of eczema but is less commonly used due to the potential for side effects.

I am an ER physician and a couple of weeks ago my nephew developed a skin condition that I had not seen before. I consulted Dr. Sung right away and sent him a picture. He replied in less than a minute with the diagnosis and treatment. I sent it to my nephews doctors who also didn't know what it was. They were able to confirm the diagnosis and start treatment right away.
Without Dr. Sun's quick diagnosis there would have been a delay of treatment and the infection could have spread to my nephews brain. Luckily it didn't and my nephew is back at home now laughing and playing as before. I went to medical school with Dr. Sung. I found true dermatology through a friend of mine who came here before and came in after discovering hair loss due to an immune reaction that the doctor was able to diagnose immediately. He started me off with some local injections and while i was quite concerned initially due to the large patch of baldness and after a few months it went away completely. Now i can't even spot where the bald soot used to be and i am truly thankful for the treatments. I'm now visiting Dr.sung for preventative hair loss treatments since this seems to be a genetic issue within the family.
